David Pruce, Director of Policy for the UK's Royal Pharmaceutical Society (RPS) told CNN: "From looking at sites that used to sell Viagra and the amount of spam emails we're having reported to us, we think that Tamiflu is now the most spammed drug on the Internet. "Since the outbreak, every Web site that used to sell Viagra is now selling Tamiflu.

We are pretty sure that the same people are making the Tamiflu as are making the Viagra." The RPS research suggests more than two million people regularly buy medicine over the Internet. However, the majority of those are from registered online pharmacies. Pruce cautioned against buying Tamiflu over the Internet, saying, "we know that half the Viagra on the Internet is fake and half the Tamiflu that's around will probably be fake as well. "The fake drugs could have anything from sugar to another drug that's similar, or often it's a lower dose of the drug, or even rat poison." His views were echoed by the UK government's chief medical officer who warned the public against panic-buying anti-swine flu drugs online. Liam Donaldson said in an interview with breakfast program GMTV that Britain had a massive stockpile of Tamiflu and would be one of the first countries to have access to a vaccine, with the first supplies arriving at the end of next month. The warning comes as it emerged Friday that a 19-year-old man has become the first person to die in London after contracting swine flu.

The teenager, who was suffering from other undisclosed serious health problems, tested positive for the virus following his death on Wednesday, according to a statement from the country's National Health Service. He is the fourth person in the UK to die from the virus. Britain's Health Secretary Andy Burnham Thursday issued a warning in the House of Commons that 40 Britons a day could die from the illness. He also said the government's emergency response had moved from containing the swine flu outbreak into the "treatment phase." The change in the government's swine flu strategy came as the World Health Organization warned that the spread of swine flu was now "unstoppable," with a total of 112 countries reporting nearly 77,201 infected people and 332 deaths since April. Dr Margaret Chan, the WHO director general, told an international conference on swine flu in Mexico: "With well over 100 countries reporting cases, once a fully fit pandemic virus emerges its further international spread is unstoppable." She added that while most deaths had so far occurred in people with serious underlying medical conditions, there were a few exceptions that caused concern.

Viagra is a well established oral medication for the treatment of erectile dysfunction (ED) and all current doses of Viagra will continue to be available to patients by prescription from their doctor. Pfizer believes that Viagra 50 mg is a suitable candidate for non-prescription supply through pharmacists in the EU and meets the criteria set out by the European Commission guideline for changing the classification of a medicinal product to non-prescription. However, in a letter to the European Medicines Agency (EMEA), Pfizer said that it has decided to withdraw the application in order to fully consider comments from the EMEA's Committee for Medicinal Products for Human Use (CHMP), recognizing that there were some concerns regarding the proposed supply of Viagra 50 mg tablets without a prescription in the EU. The CHMP noted that when Viagra is used in accordance with the currently approved prescribing information, its safety profile remains favorable.

The withdrawal of the application will enable evaluation of further information and additional data that may be required to allow any future assessments under the centralized procedure. Study of Gene Transfer for Erectile Dysfunction Shows Promise

The first human study using gene transfer to treat erectile dysfunction (ED) shows promising results and suggests the potential for using the technology to treat overactive bladder, irritable bowel syndrome and asthma, according to the researchers. “In the small pilot study, this new therapy was well tolerated and safe,” said George Christ, Ph.D., senior researcher and a professor at the Institute for Regenerative Medicine at Wake Forest University School of Medicine. “It provides evidence that gene transfer is a viable approach to treating ED and other diseases involving smooth muscle cells.” The results of the study, which included 11 men with ED, are reported online today in Human Gene Therapy. The technology was developed by Christ and Arnold Melman, M.D., when they worked together at Albert Einstein College of Medicine in the Bronx, New York. Unlike traditional gene therapy, the gene transfer approach being pioneered by Christ and Melman does not change the DNA or genetic code of cells. Instead, small pieces of DNA reach the nuclei of cells and this causes them to increase production of particular proteins.

These proteins help relax smooth muscle cells, the type of muscle found in the penis as well as in hollow organs such as the bladder. Relaxing the tissue allows the penis to fill with blood and become erect. Previous research has shown that more than 50 percent of men between 40 and 70 years old and 70 percent over age 70 may have ED. The new therapy is a potential alternative to oral medications, such as Viagra, which are not effective for an estimated 30 to 40 percent of men with ED. A possible advantage of gene transfer is that a single treatment could last for months. In the current study, improvements were maintained through the 24 weeks of study. The study was conducted from May 2004 to May 2006 at Mount Sinai School of Medicine and New York University School of Medicine. Men ranged from 42 to 80 years old with a mean age of 59. Six subjects were white, four were black and one was Hispanic. In half of the subjects, the cause of ED was diabetes or cardiovascular disease – both of which can interfere with the ability of smooth muscle cells to relax. The primary goal of the study was to determine the safety and tolerability of the new therapy. However, the results also showed that at the highest doses, men reported highly significant improvements in erectile function. The DNA segments – mixed into plasma – were injected into the corpus cavernosum, expandable tissue along the length of the penis that fills with blood during erection.

A variety of clinical and laboratory tests were used to assess safety. In addition, effectiveness was measured using the International Index of Erectile Function scale, a questionnaire that is commonly used to measure ED. Patient responses were validated by their partners. Researchers identified no safety issues with the treatment. Participants who received the highest two doses had apparent sustained improvements in ED as measured by the questionnaire. The researchers said that a larger study that includes a “control” group treated with a placebo is needed to confirm the safety and effectiveness of the treatment. Other researchers on the project were Melman, Natan Bar-Chama, M.D., with Mount Sinai School of Medicine, Andrew McCullough, M.D., with New York University School of Medicine, and Kelvin Davies, Ph.D., with Albert Einstein College of Medicine. The technology is being developed by Ion Channel Innovations (ICI), a development stage biotechnology company, in which Christ and Melman are co-founders and directing members. The therapy is known as ion channel therapy because the proteins it targets are potassium channels, “gates” within cells critical for contraction and relaxation of smooth muscle. At the Wake Forest Institute for Regenerative Medicine, Christ is continuing to pursue the therapy in collaboration with ICI, and is also exploring the potential of combining gene transfer with traditional oral medications to further increase the clinical utility of the technology.

Some say that they think twice before spending money on shoes or going out, so they have enough saved for their yearly tests, including mammograms, which can still be costly even with insurance. A number of the young women involved with Avner's organization don't want to tell their insurance companies they tested positive for the BRCA mutations, fearing rate increases or dropped coverage. "It's not an old person's disease," says Dr. Deborah Lindner, Avner's personal physician. Lindner also speaks from personal experience. The obstetrician-gynecologist, who now serves on Bright Pink's board, found out she had the genetic mutation when she was 32 and went through the same prophylactic surgery. "If a young woman finds out early on that she's at high risk, it's nothing to freak out about, but it can be something to plan for.

" Like some other high-risk young women, the sisters from Dallas bought life insurance policies in advance of their genetic tests. They feared being discriminated against if they tested positive for the BRCA mutations. They prepare for the worst and hope for the best. If they test positive, they said they would consider the preventive surgery in their later 20s. Breast cancer rates in young woman are low: about 5 percent of breast cancers occur in women younger than 40. But, when it does happen, the cancer is more likely to spread faster, be of a higher grade (the cells look and behave more abnormally) and be less receptive to chemotherapy and other treatments. Psychologically, it can be especially shocking and difficult for young women who have to deal with a "grown-up" medical issue at a time of life usually reserved for family and career. "If I don't find Mr. Right by 35, I am freezing my eggs," Avner says, noting her increased risk of ovarian cancer. A preventive mastectomy doesn't guarantee that she won't develop ovarian cancer. In fact, it doesn't even eliminate the risk of breast cancer.

And what about the prospects of dating and finding Mr. Right? "It's just as hard as it is for any woman in her 20s. But, I am in a good place right now. I know myself and I'm comfortable with who that is," Avner tells the group in a calm, confident tone. "Some guys Google me before we go out. They're immature and tell their friends how [my breasts] look and feel, but those guys aren't for me," Avner says taking a sip of her drink and looking out into the bustling line of cabs arriving at the surrounding bars. "I mean, after all, what difference does it make? It's just a boob."

It's Saturday night. Three young women are dressed to the nines at a trendy bistro on Rush Street in downtown Chicago. They're having drinks outside on the kind of summer night that makes you fall in love with the city. Lindsay Avner (left, with her mother) had a mastectomy after testing positive for the BRCA gene mutation. "They call this 'Viagra triangle,'" Lindsay Avner says, to peals of laughter. On this night, these twentysomethings aren't thinking about flirting, though. They are thinking about issues most women their age don't: life insurance and freezing their eggs. Tonight Avner is talking to two sisters from Dallas, Texas, about their imminent genetic testing to see whether they have the BRCA gene mutations that will predispose them to developing breast and ovarian cancer. Avner, 26, knows what they're going through. She tested positive for the gene three years ago.

Women with the BRCA gene mutations have a 60 to 80 percent chance of developing breast cancer -- at least five times higher than the general population -- according to the American Cancer Society. Additionally, these women have up to a 60 percent chance of getting ovarian cancer in their lifetime. "I know you're scared. It's a scary thing," Avner says. She has told her story dozens, even hundreds, of times but says she never tires of it. "I keep telling myself that it's better to know and be ready." Women with the inherited mutations have an increased risk of developing these cancers at a young age, before menopause, and often have multiple close family members with the disease, according to the National Cancer Institute. "The funny thing is that nothing in your body has changed the day you hear the results," Avner says, reflecting on the experience of hearing a positive test result. "You were either born with it or you weren't. You are just going armed with more knowledge." That genetic knowledge convinced Avner to undergo a prophylactic double mastectomy when she was 23.

The surgery involves removing as much of the at-risk tissue as possible, in order to reduce the chance of developing cancer. A year later she started an organization to educate young women about breast cancer and preventive health, Bright Pink. "It's something our parents and grandparents didn't have available," Avner said. Every female member of the family, from aunts to her grandmother, died from the disease. Her mother is the only survivor, but endured four bouts with breast and ovarian cancer. "I wanted to know, so my future kids don't suffer like I did, seeing my mom going through chemo and struggling with it." Most young women involved with Bright Pink have had family members with the disease. Because of their increased risk, they are remarkably conscious of their health.

EAU 2007 - Innovations in Men’s Health: Real-Life Satisfaction for Men and their Partners

The CONFIRMED study avoided biases seen in other comparative trials by consciously creating an unbiased methodology that included 13 separate criteria. Beyond the randomised, doubleblind, cross-over design, this trial additionally used equivalent drug doses, randomised the treatment sequence, used a neutral consent form, assessed preference after each treatment period and, importantly, did not eliminate previous non-responders or only include naIve patients. For many men, testosterone levels decline naturally as they age. This effect goes unnoticed and is often regarded as a natural phenomenon of aging. However, low testosterone levels can have a serious effect on the quality of life of men and is commonly associated with a decrease in libido, lean body mass and bone density and an increase in visceral fat as well as disturbances in well-being and mood. Eight centres in Austria, Finland, Germany, Ireland, Spain, Sweden and UK recruited 365 patients with decreased testosterone levels and hypogonadal symptoms in this multi-centre, placebo controlled, double-blind, randomised studl. This study found that treatment with Testogel, a transdermal1 % hydroalcoholic gel restored testosterone levels in hypogonadal men to normal range. Testogel treatment had a statistical significant effect on lean body mass versus placebo. Body composition was also favourably influenced by Testogel treatment compared to placebo. Nebido, an intramuscular depot formulation of testosterone undecanoate, can provide safe and effective drug treatment with high satisfaction for men.

An open label prospective, one arm study in 16 centres in Germany included 96 hypogonadal patients, with a mean age of 48.6 years. The men received 6 injections of Nebido over a period of 63 weeks. The results showed that Nebido not only improved the lipid pattern (reduction of total cholesterol and LDL cholesterol) as well as glycaemia control (levels of blood sugar) but also improved psychological, somatic, and sexual symptoms of testosterone deficiency. There were no clinically relevant side effects and 92.5% of patients assessed treatment with Nebido as satisfying.?

Couples agree that Levitra provided significantly greater satisfaction versus placebo and that satisfaction is achieved by not only to the man, but also to his partner. The Levitra CONFIRMED study was undertaken to compare the efficacy and safety of vardenafil (Levitra) to that of sildenafil (Viagra) when used on demand in a population of men with ED. The CONFIRMED study demonstrates the non-inferiority of Levitra in comparison to Viagra for overall preference and nominal statistical superiority of Levitra to Viagra for several frequently used efficacy variables. The results in a total of 1057 men who were randomised to treatment, showed that 38.9% of men expressed an overall preference for Levitra over Viagra (34.5%) and 26.6% of patients had no treatment preference. The difference found was not statistically significant. Regarding secondary measures, Levitra achieved better response for men responding to the Sexual Encounter Profile questions 2 and 3; 83.9% of men taking Levitra responded positively to SEP2 (p=0.0389) compared with 82.28% for Viagra and 74.4% of men preferred Levitra in response to SEP3 (p=0.0038) against 71.6% for Viagra.

Acute type 5 phosphodiesterase inhibition with sildenafil enhances flow-mediated vasodilation in patients with chronic heart failure.

OBJECTIVES: To determine the acute effects of type 5 phosphodiesterase inhibition with sildenafil on flow-mediated vasodilation in the brachial artery of patients with chronic heart failure. BACKGROUND: Impaired endothelium-dependent, flow-mediated vasodilation in patients with heart failure is partly attributable to hyporesponsiveness of cyclic guanosine monophosphate (cGMP) mediated vasorelaxation effector mechanisms in vascular smooth muscle. The effect of inhibition of cGMP degradation with sildenafil, a specific type 5 cGMP phosphodiesterase inhibitor, on flow-mediated dilation in heart failure is unknown.

METHODS: Flow-mediated vasodilation after release of 1, 3 and 5 min of transient arterial occlusion was measured in the brachial artery with high resolution two-dimensional ultrasound imaging in 48 patients with chronic heart failure before and 1 h after randomized, double-blind assignment to a single oral dose of sildenafil 12.5, 25 or 50 mg or matching placebo.

RESULTS: In response to oral administration of a single dose of study drug, the change in flow-mediated vasodilation after release of 1, 3 and 5 min of arterial occlusion was significantly greater in patients receiving sildenafil 25 mg (3.3 +/- 1.9, 3.8 +/- 1.8 and 4.0 +/- 1.8%, respectively, p < 0.05) and patients receiving sildenafil 50 mg (3.7 +/- 1.3, 4.1 +/- 1.1, 3.9 +/- 1.3%, respectively, p < 0.05) than that of patients receiving placebo (0.7 +/- 1.1, 0.2 +/- 1.2, 0.6 +/- 0.8%, respectively). CONCLUSIONS: Acute type 5 phosphodiesterase inhibition with sildenafil 25 and 50 mg increases endothelium-dependent, flow-mediated vasodilation in patients with chronic heart failure when compared with placebo.

Raising the issue of Viagra costs- who should pay?

What's your opinion?Imagine a new drug that could restore some lost physical ability, at least for a few hours. Then imagine that this new drug costs $10 a dose, and could be used by millions of people. Viagra fits this description, of course, and its magic is to restore virility to impotent men-albeit for a few hours at a time. Viagra is predicted to be a billion-dollar seller for Pfizer in its first year of sales, and that means someone is paying for all those $10 pills. Some managed care companies have announced that they will not pay for Viagra based on its high cost, but it is hard to imagine them making a similar decision about an equally expensive drug that cured a specific type of cancer or reversed paralysis. Would there be hesitation to cover a drug that would restore the use of paraplegics' legs, even if each pill cost $10 and its effects diminished after a few hours? No doubt we would consider it a miracle and a bargain. So why not Viagra? Who needs lifestyle drugs? Part of the motivation for denying payment for Viagra is the perception that it doesn't cure or even treat illness or disease, and that the functions it temporarily restores are not life-threatening or critical enough for it to make sense to pay for it.
